Health Information Forms
Upon entering the office, we have some health information forms for you to fill out. This information will provide us with general information about yourself and your condition. You can also fill out the forms in advance and bring them with you to your first visit by visiting our Forms section of our website and print them ahead of time.
Once your forms have been completed, you will have a consultation to discuss your health-related problems, concerns, as well as potential treatment options.
History and Examination
After your consultation, we will ask you various questions related to your condition. Next, a number of specialized (non-invasive) computerized spinal scan tests will help determine which soft tissues such as nerves and muscles are affected and to what extent. Also performed are several orthopedic and neurological tests along with a range of motion and postural exam. example of the computerized spinal scan
Your specific condition may require us to refer out for x-rays to either rule out more serious conditions, confirm examination findings and assist the doctor in developing the most effective treatment plan for you.
Physician or other Diagnostic Procedure Referrals
If the doctor determines that one or more of your conditions requires a referral to another healthcare specialist for additional testing or concurrent treatment or to a facilty for additional diagnostic tests, our office will make the referal and coordinate your care between our office and theirs to provide the best of care for you and the treatment of your condition.
